Complimentary Movement Assessments with the Spooner Team

Seeking a physical therapist first for your injury, aches and pains can save you time and money. It may even get you moving better, sooner than you thought possible!

During the assessment, you and your therapist will discuss your concerns, take a look at how you move, and assess your level of discomfort. Together, we will create a plan to help you return to moving and feeling your best both on and off the field!

If it turns out you need to see a specialist outside of Spooner, our team will help get you connected with the best provider to address your concerns.

AUTHORIZATION TO ASSESS AND TREAT

All procedures will be thoroughly explained to the athlete before they are performed. There are certain inherent risks with injury screens, movement assessments and treatment. These could increase the current level of pain or discomfort or could aggravate an existing injury. There is also a possibility of experiencing a new injury, but the risk is small. The therapist/trainer will take every precaution to ensure the athlete is protected, and the athlete will be able to stop the assessment and/or treatment at any time. Based on this information, I understand and authorize Spooner to perform appropriate assessment and/or treatment techniques based on a general assessment of my or my child’s condition.

MEDIA RELEASE

I grant permission to Spooner and its agents and employees the irrevocable and unrestricted right to reproduce the photographs and/or video images taken of me for the purpose of publication, promotion, illustration, advertising, or trade in any manner or in any medium. I hereby release Spooner and its legal representatives for all claims and liability relating to said images or video. I waive my right to any compensation.
